Complete the Service Record Request form 

‣ If you are leaving at the end of your contract, Human Capital will begin processing service record requests in July. Service records will be processed in the order they are received.
‣ During peak season, allow 30 days for your request to be processed.

‣Once you meet with your principal or supervisor, please leave your Keys/Badge/proxy with your campus or department. Those items will remain on campus or department to be reassigned to a new employee.

‣Return technology equipment to Central Admin Office. This includes laptops, iPads, and dongles that were provided. Reserve a time to drop off your equipment through an appointment. Drop-off will be outside the main entrance to central office.
