Employee Self Help Forms
These forms may be downloaded and emailed by any MISD employee as needed to firstname.lastname@example.org. Below are short descriptions of each.
MISD D2-A Demographic Data: Current employees may use this form to update personal information such as address and phone number. If the employee has a change in name request, this form CANNOT be emailed. This form will need to be submitted in person to HR.
MISD D10-B Employee Transfer Request: Employees seeking an employment transfer to another campus or department must complete this form PRIOR to interviewing.
MISD D14-A Separation Checklist: Employees leaving MISD should complete this form after submitting a letter of resignation to their supervisor.
MISD Request for Records: Employees wishing to obtaining records filed in Human Resources should complete this form.
Employee Grievance Level ONE: Employees wishing to file a compliant may complete this form.
Employee Grievance Level TWO: Employees wishing to appeal the decision of Level One or the lack of a timely response after a Level One conference may complete this form.
Employee Grievance Level THREE: Employees wishing to appeal the decision of Level Two or the lack of a timely response after a Level Two conference may complete this form.
