Conflict of Interest Disclosures/Criminal History Forms

***Conflict of Interest Disclosures***
 
NOTICE TO VENDORS:
Local Government Code, Section 176.006, requires that persons or agents who enter or seek to enter into a contract with Manor ISD for the sale or purchase of property, goods, or services are required to file a Conflict of Interest Questionnaire (Form CIQ) with the Purchasing Department. Each covered person, entity, or vendor who seeks to or who contracts with MISD is responsible for complying with any applicable disclosure requirements. MISD will post the required completed questionnaires on this website.
 
The Conflict of Interest Questionnaire must be filed:
  • No later than the seventh business day after the date that the Vendor begins contract discussions or negotiations with the government entity, or submits to the local governmental entity an application, response to a request for proposals or bids, correspondence, or another writing related to a potential agreement with Manor ISD.
  • No later than September 1 of the year in which an activity described in Section 176.006(a), Local Government Code, is pending, and not later than the seventh business day after the date of an event that would make a statement in the questionnaire incomplete or inaccurate,when the Vendor has filed a previous completed questionnaire.
 
A vendor commits an offense if the vendor knowingly violates Section 176.006, Local Government Code. An offense under this section is a misdemeanor. This requirement will be addressed in all requests for bids or proposals and new vendors requesting to do business with the district.
 
A vendor is not required to file an updated completed questionnaire in a year if the person has filed a questionnaire on or after June 1, but before September 1, of that year.

***Notification of Criminal History***
 
Texas Education Code, Section 44.034 requires that a person or business entity that enters into a contract with a school district must give advance notice if that person or an owner or operator of the business has been convicted of a felony. Contracts may be terminated for failure to give notice or if conduct is misrepresented. A certification page is included in all requests for bids and proposals but if a vendor is doing business by Purchase Orders, a notification form is still required for such convictions.
